TriFire Assessor sanctioned by The Institution of Fire Engineers (IFE) following disciplinary action which confirmed breaches of the IFE Code of Conduct and Ethical Principles.

The sanctions include a six-month suspension of IFE membership, registration, and volunteering roles, effective from August 21, 2024.

TriFire Assessor Sanctioned - Background

The TriFire Assessor Sanctioned was Adam Kiziak. The case arose from allegations that Mr. Kiziak failed to uphold professional standards and demonstrate competence in his work as a fire engineer.

Specific breaches include failing to maintain adequate professional indemnity insurance, which is essential for protecting clients and ensuring accountability in the profession.

Furthermore, he failed to disclose exclusions in his insurance policy that could prohibit certain work activities, a serious lapse that jeopardizes professional integrity and safety.

Additionally, Mr. Kiziak was found to have operated outside his professional competence, failing to exercise reasonable skill and care in his work. These actions breached the IFE's core principles of honesty, integrity, and impartiality, as well as the ethical requirement for accuracy and vigor in professional conduct. TriFire carries out fire risk assessments, EWS1 surveys and fire engineering.

The IFE’s Professional Standards Committee reviewed the findings and upheld the disciplinary panel’s recommendation to impose the sanctions. The committee emphasized the importance of these actions in maintaining the reputation and integrity of the fire engineering profession, as well as ensuring public trust.

Sanctioned TriFire Assessor - Consequences

With this TriFire Assessor sanctioned concerns will be raised about the work completed by Mr. Kiziak prior to this judgement by the IFE.

In an article reporting on this case, Inside Housing described Mr. Kiziak as ‘a prolific fire risk assessor since setting up business in 2020’. The IFE’s judgment must call into question any EWS1 form signed by Mr. Kiziak and relied on by building owners and lenders.

Not only was Mr. Kiziak’s work for TriFire found to be incompetent, but he reportedly did not have appropriate PII cover for his work.

Since the IFE imposed these sanctions, further worrying events regarding Mr. Kiziak have come to light.

In an open letter on LinkedIn, Awwal Salisu reported the fraudulent use of his credentials to issue EWS1 forms. This was done without his knowledge or authorization. The extent of fraudulent forms issued by TriFire is not currently known. However, combined with the issues found by the IFE, all of TriFire’s work since it began trading in 2020 could be in question.

This news must be very worrying for any occupier of a building assessed by Mr. Kiziak or TriFire.
